RN case manager of patient services and medical care works collaboratively with other disciplines to teach, assist, and support the patient/family with the dying process. Organization needs fluctuate, maybe need to work in different shifts or locations. In addition excellent in verbal/written communication, the candidate will demonstrate experience in developing and maintaining relationships with patients, families and medical providers and have the ability to work well with an interdisciplinary team.
Primary Responsibilities:
- Assessment and documentation appropriateness of hospice at initial and ongoing assessment visits.
- Consult with Hospice Physician and/or clinical manager.
- Provide information on hospice care programs to referral sources patients and families
- Complete admission documentation with patients and families as required.
- Provide direct nursing care to patients for developing the best level of comfort and quality of life. Assist, direct and manage the physical care of the patient by providing medication, diet and other instructions as prescribed by the physician, Primary Care Nurse and Clinical Manager on telephone contacts or home visits
- Collect sufficient necessary information to function effectively in the Hospice setting where the patient/family/caregiver need care Provide professional nursing care by using all resources of the nursing process.
- Provide professional nursing care by using all resources of the nursing process.
- Apply appropriate preventative and therapeutic nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel
- Compile and submit accurate and relevant clinical notes regarding the patient’s condition and care given.
- Arrangement of equipment and materials needed from community resources. Use supplies and equipment effectively and efficiently
- Teach techniques appropriately to the patient/family/caregiver self-care.
- Work collectively with the other Hospice team members.
Qualifications
- Current NC or Multistate RN licensure
Preferred Skills/Experience
- BSN, CHPN preferred
- Proficient in electronic documentation
- 2-4 years’ experience in hospice or home care
Extensive experience in:
- Excellent communication, organization and customer service skills
- Strong desire to provide compassionate, end-of-life care

- Thursday: 9:00 - 17:00
- Friday: 9:00 - 17:00
- Saturday: 10:00 - 16:00
- Sunday: Closed!
